Why Cats' Kidneys Fail: The 20-Year Study Almost No Owner Has Heard Of
A lab in Tokyo spent twenty years working out why kidney disease kills so many cats. What they found is in every cat alive — and it explains why the things we do to protect them never reach the problem.

Here is a fact most cat owners have never been told.
Your cat was born with a broken part.
Not a rare one. Not something she got from a bad breeder. Every domestic cat on earth has it. So does every lion. So does every tiger.
It's one protein. Every mammal has this protein — you have it too. It has one job. To clean waste out of the kidneys.
Yours works.
Hers has never worked. Not once. Not in her, and not in any cat that has ever lived.
For a long time nobody knew this.
Then a scientist in Tokyo spent twenty years working out why.
A Protein Found by Accident
His name is Toru Miyazaki.
In the 1990s he was working at a lab in Switzerland, going through immune genes to see what he could find. He called it treasure hunting. And he found a protein that nobody thought was important.
He named it AIM.
It had nothing to do with cats. It wouldn't for another twenty years.
Then, back in Tokyo, a vet friend said something to him in passing.
Roughly one cat in three develops kidney disease. It kills more cats than anything else — and no other animal comes close.
And Miyazaki asked a question nobody in his field had thought to ask.
Not how to treat it. Why does it happen at all?
Every treatment out there was built to slow the damage down after it showed up. Nobody had gone looking for the thing that starts it.
So he did.
It Wasn't a Disease. It Was a Design Flaw.
Start with how a kidney works.
Inside your cat's kidneys are thousands of tiny tubes. Blood gets cleaned as it passes through them.
Cells inside those tubes die all the time. That's normal. It happens in every animal, every day. The dead bits have to be swept out.
If they aren't swept out, the tube clogs. A clogged tube stops working. Enough clogged tubes, and the kidney starts to fail.
So the body has a cleaning system. AIM is the signal that starts it. AIM sticks to the dead bits and marks them, like a tag on a bag of trash. Then the body knows to come and take it away.
But AIM doesn't float around loose in the blood. It rides on something.
Think of it like a ship. The ship is an antibody called IgM. AIM is the crew, waiting on the deck. When the kidney calls for help, the crew is supposed to jump off the ship, get down into the tube, and start clearing.
In cats, they never get off the ship.
Miyazaki's team compared the AIM protein in cats to the same protein in other animals. In cats they found something no other animal had — a tiny cluster of amino acids sitting in exactly the wrong place.
It basically glues the crew to the deck.
So the alarm goes off. The crew is right there, ready. But they can't leave.
The trash never gets collected.
They proved it the hard way too. They bred mice carrying the cat version of AIM. Those mice died of kidney damage far more often than normal mice. Same kidneys. One changed protein.

It's in every house cat. It's in lions. It's in tigers. It has been there for as long as there have been cats.
Which means it was never something your cat developed. And it was never something you could have prevented.

Here's the cruel part.
Waste that never gets cleared doesn't just sit there. It builds. And as it builds, it makes things worse.
- The crew stays stuck. Waste that should be swept out stays in the tubes.
- Tubes clog. A clogged tube dies. It never comes back.
- The rest work harder. Fewer working tubes means more strain on the ones left.
- And still nothing shows. Cats are born with far more kidney than they need. So the damage runs for years while she looks completely fine.
By the time the blood test moves, most of it has already happened.
She wasn't hiding it from you.
She had nothing to show you.
What owners actually notice first
Almost never a dramatic symptom. It is usually one of these, and it is usually put down to age:
- She drinks a little more water than she used to
- The clumps in the litter box got bigger
- She leaves dinner half-finished, and she's got fussier in general
- More hairballs than there used to be
- Her coat isn't as soft under your hand as it was two years ago
- She stopped getting up on the fridge, and you can't remember when
- She sleeps more. She's quieter.
None of these are early signs. By the time they show, the silent years are already behind her. That is why a blood test at a well visit — before anything looks wrong — is worth more than any symptom you can watch for.
If she has nothing to show you, then watching for a sign was never a plan.
Why the Usual Advice Can't Reach It
Now here's why this matters for what you're already doing.
Almost every owner gets handed the same list. A water fountain. Better food. A taurine supplement. Yearly check-ups.
All of it is good care. None of it is a waste. But not one of those things touches the cleaning system.
The Water Fountain
More water helps. It thins the urine and takes some strain off. But it can't remove what's already stuck inside the tubes. Running more water through a pipe doesn't clean off what's stuck to the wall.
The Premium Food
Better protein, less phosphorus, fewer additives. All of it means the kidney has less work to do. But less work is not the same as a clean-out.
The Taurine
A lot of owners give taurine for kidneys. Taurine matters — cats can't make enough of their own, and it protects living cells. But protecting a living cell and clearing away a dead one are two different jobs.

This is the best tool most vets have, and it should be used. It slows things down and manages what's already there. It manages. It doesn't repair.
Look at the pattern.
- The fountain dilutes. It does not clear.
- The premium food lightens the load. It does not clear.
- The taurine protects living cells. It does not clear.

The Treatment That Works
Miyazaki didn't stop at finding the problem. His team built a treatment for it.
It's called recombinant AIM. It's an injection. It does the job her own protein can't.
In 2026 they published the results in The Veterinary Journal. Twenty-six cats with advanced kidney disease.
I don't use the word breakthrough often. That's one.
Now the hard part.
You can't get it.
The trials are done. It was filed with Japan's Ministry of Agriculture in April 2026. Review takes six to twelve months. Best case, it reaches vets in Japan somewhere between late 2026 and early 2027.
Then it has to clear regulators in every other country, one at a time. For the United States, that's years, not months.
And it nearly didn't happen at all.
In 2021 the money ran out. The company funding the work pulled out when the pandemic hit the economy. The trials stopped.
A Japanese news agency ran an interview about it. Within hours, three thousand people had sent donations to the University of Tokyo. Within a few days it was ten thousand — more than the university normally receives in a whole year.
By that September they had given ¥207 million. Around $1.9 million. All of it from people who own cats.
Miyazaki said it was the first time he understood how much his work was being waited on.
Owners have been waiting on this a long time. On the kidney forums you can watch what the waiting costs. One of them wrote:
That's a man whose cat ran out of time while the science sat in limbo.
And there's one more line in that 2026 paper that almost nobody has picked up on. I think it's the most important sentence in this article.
The line everyone missed
The cats who came in with less waste already built up survived at 100% — with no treatment at all.
How much had piled up before anyone stepped in decided who lived. Not the drug. The starting point.
So the question was never only what do we give her when she's sick.
It's how much is she carrying by the time we get there.
Which Is Why Somebody Decided Not to Wait
Here's the obvious thought: just give her AIM.
You can't. AIM is a protein. Swallow it and the stomach breaks it apart long before it reaches a kidney. That's exactly why the treatment has to be an injection.
But replacing the protein was never the only route.
Miyazaki left the University of Tokyo and founded the Institute for AIM Medicine. And on their site, they draw a line almost nobody outside the field has heard:
So here is the opening. You can't give her the protein itself. But you can give her the things that help her body get more out of the protein she already has.
Their own position is that giving AIM directly suits a crisis — a sick cat, now. Waking up the AIM she was already born with suits the slow version. Or in their words: prevention.

Is this the AIM injection?
No — and don't buy from anyone who says otherwise. The injection is a veterinary drug, it's under review in Japan, and you can't buy it anywhere. This is an oral formula built on the activation pathway, the approach Miyazaki's own institute describes as suited to prevention.
Is it too late for my cat?
If she already has a diagnosis, this doesn't replace anything your vet has prescribed. Keep the renal diet. Keep the fluids. This goes alongside them. The research does suggest the earlier you start, the more it matters.
Will she actually eat it?
It's freeze-dried tuna. Most cats come running. That's the whole reason that base was chosen — because the most common reason renal supplements fail isn't that they don't work, it's that the cat refuses them.
Isn't this the same as the amino acid supplement I already give?

I keep going over it at night. Was there a sign? Did I miss it? I did everything right, so why does it still feel like this is my fault.
Karen — this is the part of the research I most wanted people to see. The damage runs silent for years, and there is nothing on the outside to notice. There was nothing to miss.
Cats are so good at hiding how they feel. Mine just started getting more hairballs. That was it. That was the only sign. Then one day she collapsed.
Hold on. Miyazaki said in an interview years ago that AIM should not be taken by mouth, because the digestive system breaks it into its component amino acids. So any oral AIM product is a scam, isn't it?
You are right, and the article says the same thing. You cannot swallow AIM. Anything sold as "oral AIM" is either confused or lying, and I would walk away from it.
What is in this article is a different route — the one his own institute describes, where you take things by mouth that help the body get more out of the AIM it already has. That distinction is the whole point, and it is worth holding people to it.

I am just praying mine holds on long enough for the injection to get here. Reading that it is years away for us was hard.
I have been through probably 50 different foods and treats. He sniffs it and walks away. After a while you stop caring what is in it, you just want him to eat something.

It will not undo damage that is already done, and it does not replace anything your vet has prescribed — keep the diet and the fluids. What the research says is that the earlier the starting point, the more it matters. That is an argument for starting now rather than an argument that it is too late.
